TALL CITY BLACK GOLD CHEER SQUAD PARTICIPATION WAIVER

In consideration of being allowed to participate in any cheerleading, dance, performance, practice, workout, gameday, promotional appearance, or related activities (collectively, the “Activities”) with the Tall City Black Gold Cheer Squad (“TCBG Cheer”), the undersigned acknowledges and agrees as follows:

1. ASSUMPTION OF RISK I understand that participation in cheerleading and dance activities involves inherent risks, including but not limited to sprains, fractures, concussions, falls, collisions, physical contact injuries, illness, permanent disability, or death. I knowingly and voluntarily assume all risks associated with participation.

2. MEDICAL RESPONSIBILITY Tall City Black Gold does not provide medical or health insurance coverage for cheer participants. I am solely responsible for maintaining adequate health, accident, and hospitalization insurance coverage.

3. RELEASE OF LIABILITY I release, waive, discharge, and agree not to sue Tall City Black Gold, its owners, directors, coaches, staff, volunteers, sponsors, venue operators, affiliates, and representatives from any and all claims arising from injury, illness, or death related to participation in the Activities.

4. INDEMNIFICATION I agree to indemnify and hold harmless Tall City Black Gold and its representatives from any loss, liability, damage, or cost incurred as a result of my participation.

5. EMERGENCY MEDICAL AUTHORIZATION I authorize Tall City Black Gold representatives to obtain emergency medical treatment on my behalf and accept full responsibility for all associated costs.

6. ACKNOWLEDGMENT I certify that I have read this agreement, fully understand its terms, and sign it freely and voluntarily.
