Please note, this specific volunteer opportunity requires by law the completion of 3 main clearances. We highly encourage you to complete your clearances and upload copies of your documentation to this form, however you can also complete this form without them:

PA Child Abuse History Clearance (CY-113): Free for volunteers.
PA State Police Criminal Record Check (SP4-164): Free for volunteers.
FBI Criminal History Record (Fingerprinting): $22.95 for volunteers.
