Enrolment

I hereby enrol myself/my child as a member of the Mark Wainman Karate Centre for this year 2026.

I acknowledge that the dojo term fees, as set out in the 2026 fee structure, are payable in advance, and are not dependant on how many classes will be attended or not.

I also acknowledge that training at the Mark Wainman Karate Centre requires myself/my child to be annually affiliated to SA JKA for a fee of R950 (coloured belts) / R1100 (black belts) for this year 2026.

Liability Waiver

I, the undersigned, in my capacity as father/mother/guardian/my personal capacity, do hereby indemnify and hold harmless the MARK WAINMAN KARATE CENTRE, the INSTRUCTORS or anyone else involved in any way with the DOJO, from any death, injury, disabiliy and all claims, causes of action, losses, damages, costs, expenses (including but not limited to attorney's fees), either known or unknown, now existing or arising in the future that I may have of whatever kind or nature, which I/my child may suffer as a result of my/his/her* participation in classes or any dojo event. I acknowledge that the participation by myself/my child in any of the classes/events is on my/his/her free will and is at own risk.

Publicity Consent

I hereby give consent to the Mark Wainman Karate Centre to take and use my/my child's image, likeness, and/orsound of my/my child's voiceas recorded in any photograph, photographic image and/or video taken during training sessions and/or competitions or related activities, in Mark Wainman Karate Centre's publications, including but not limited to training materials, website, newsletters, social media pages, pamphlets and marketing materials.

I hereby grant the Mark Wainman Karate Centre the irrevocable and unrestricted right and permission to copyright, in its own name or otherwise, and to use, re-use, publish or re-publish the photograph, photographic image or video recording taken of me/my child or in which may be included as described herein before, in whole or in part, or composite or distorted in character or in form, without restriction as to changes or alterations, or reproductions thereof in colour or otherwise, in conjunction with my/my child's own name made through any medium by the Mark Wainman Karate Centre for illustration , promotion, or any other purpose whatsoever.

Additionally, I waive any right to royalties or other compensation arising from or related to use of my/my child's image, likeness, and or sound of my/my child's voice as recorded in any photograph, photographic image and/or video.
