Adoption Acknowledgement & Agreement

By signing this Adoption Completion Form, the Adopter confirms and agrees to the following:

• Terms & Conditions Acknowledgement The Adopter acknowledges that Adopter already reviewed and signed the Shelter’s Adoption Application Form and fully understand all associated terms and conditions. The Adopter agrees to continue to follow and comply with all adoption terms, policies, and guidelines set by the Shelter.
  

• Ongoing Responsibility & Proper Care The Adopter agrees to provide lifelong, humane, and responsible care for the adopted pet, including proper food, shelter, veterinary care, exercise, and companionship.
  

• Prohibited Actions The Adopter agrees that they will NOT, under any circumstances:

  • Surrender or transfer the adopted pet to another shelter, rescue, organization, or individual without prior written consent from this Shelter;

  • Give, sell, rehome, or abandon the pet to another person without approval from the Shelter;

  • Neglect, abuse, or mistreat the pet in any way.
    

• Right of Repossession The Adopter understands and agrees that if any adoption term or condition is violated—including but not limited to unauthorized transfer, surrender, neglect, or abuse—the [Maplelatte Animal Shelter Society] reserves the full right to reclaim and take back the adopted pet without refund of any adoption fees.

  
  

• ***Spay and Neuter Requirement*** If the adopted pet is a female puppy/dog that has not yet been spayed, the Adopter agrees to have the pet spayed by a licensed veterinarian when age-appropriate.
  

  The Adopter MUST provide a copy of the spay certificate or veterinary record to Maplelatte Animal Shelter Society after the surgery is completed.
  

  Allowing the adopted female puppy/dog to become pregnant before being spayed is a violation of this Adoption Agreement. In such cases, [Maplelatte Animal Shelter Society] reserves the right to repossess the pet without refund of any adoption fees.
  

• Adoption Updates & Follow-Ups The Adopter agrees to provide photo updates and brief written updates about the pet’s wellbeing to [Maplelatte Animal Shelter Society] at the following intervals after adoption:

  • One (1) week after adoption

  • One (1) month after adoption

  • Three (3) months after adoption

Return Policy

If the Adopter is genuinely unable to continue caring for the pet due to unforeseen or significant circumstances, the pet must be returned to Maplelatte Animal Shelter Society. The pet may not be sold, rehomed, given away, surrendered to another shelter or rescue, or transferred to any individual or organization without the Society's prior written consent.

The Society reserves the right to determine whether a return request is reasonable and justified. Returns based solely on convenience, lifestyle changes, loss of interest, or other avoidable circumstances may be declined. The Society may also require the Adopter to work with the shelter to explore reasonable solutions before accepting the return.

Before any return is accepted, the Adopter must:

• Complete the Society's Surrender Form in full;

• Follow all surrender procedures and instructions provided by Maplelatte Animal Shelter Society; and

• Pay the applicable intake/surrender fee, unless waived in writing by the Society.

Failure to comply with this policy or transferring the pet without the Society's written approval shall constitute a breach of this Adoption Agreement and may result in legal action or other remedies available under applicable law.
