MobileX/ T-100 Repair Service Agreement

I understand that the unit will be evaluated to determine warranty status and the extent of repair required. If the unit is found to be outside of warranty coverage, any applicable bench or repair fees will be communicated to me for approval before any charges are processed.*

I agree to pay a non-refundable Bench Fee of $450 if my unit is determined to be outside of warranty coverage. This fee covers the initial evaluation to determine the extent of the repair and whether the unit is repairable. Payment of this fee is required before an RMA (Return Merchandise Authorization) will be issued for sending in the unit.*

I authorize Denterprise International, Inc. to charge the approved repair fee after I have reviewed and approved the final repair cost provided following the evaluation. Repair costs may range from $600 to $1,800, depending on the required repairs.*

I understand the payment retention policy. If the credit card payment is declined for any reason, Denterprise International, Inc. will retain possession of the X-Ray Generator until the repair fee is paid in full. If payment is not received within 90 days, the unit will be considered forfeited and become the property of Denterprise International, Inc.*
