Volunteer Acknowledgment Statement

   I am volunteering my time and services willingly and without expectation of compensation, benefits, or future employment.

2. Compliance with Policies

   I agree to follow all guidelines, instructions, and safety procedures provided by West Slope Foundation staff and representatives during my volunteer activities.

3. Assumption of Risk

   I understand that volunteering may involve certain risks and I accept full responsibility for my participation. I release West Slope Foundation, its staff, and affiliates from any liability for injuries or damages that may occur during volunteer activities, except as prohibited by law.

4. Confidentiality

   I agree to maintain the confidentiality of any sensitive information I may encounter while volunteering.

5. Media Release

   I grant permission for West Slope Foundation to use photographs or videos taken during volunteer activities for promotional purposes, unless I opt out in writing.
