Liability Waiver and Release

Please read this waiver carefully and complete all required information before participating in activities.

1. Acknowledgement of Activities

I understand that participation in activities at RiverTown Family Fun Zone ("RiverTown") may include, but is not limited to:

• Indoor Playground structures, slides, climbing features, and elevated platforms
• Toddler Soft Play Area including padded climbing structures and age‑appropriate activities
• GellyBall games and events, including the use of GellyBall blasters, ammunition, and safety equipment
• Mini Golf including walking on uneven surfaces and interacting with obstacles
• Open Play Areas including indoor recreation and mixed‑age interaction
• Interactive Arcade Games including electronic, virtual, and mechanical devices
• General facility use including walking throughout the premises and participation in events

• Axe Throwing when offered. inthe open play area.

I understand these activities involve physical exertion, running, jumping, climbing, projectiles, and interactive play with inherent risks.

2. Assumption of Risk

I acknowledge that participation includes inherent and unavoidable risks, including but not limited to:

• Slips, trips, and falls
• Collisions with other participants or equipment
• Strains, sprains, or other injuries
• Impact from GellyBall projectiles
• Equipment malfunction or misuse
• Unpredictable behavior of other participants
• Minor cuts, bumps, or bruises
• Serious injury including fractures, concussion, or in rare cases, paralysis or death

By signing below, I voluntarily assume all such risks for myself and/or any minor child under my care.

3. Release of Liability

In consideration for being allowed to enter and use RiverTown, I hereby release, waive, discharge, and hold harmless RiverTown Family Fun Zone, its owners, officers, employees, contractors, volunteers, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of any injury, loss, or damage occurring on the premises or during participation in any activity.

This release applies to the fullest extent permitted by law, regardless of whether the injury or damage is caused by negligence or otherwise.

4. Medical Treatment Authorization

I authorize RiverTown staff to secure basic first aid and/or emergency medical care if necessary. I understand that I am responsible for any resulting medical expenses.

5. Responsibility for Minors

If signing for a minor:

• I certify that I am the minor's parent or legal guardian
• I accept full responsibility for supervising the minor
• I agree to indemnify and hold RiverTown harmless from any claims brought by or on behalf of the minor

6. Rules & Safety Compliance

I agree to comply with all posted rules and verbal instructions including, but not limited to:

• No running in designated areas
• Proper use of safety/protective equipment
• Respecting age and height restrictions
• Wearing protective eyewear during GellyBall
• No roughhousing or unsafe behavior
• Socks required in play areas
• Following all staff instructions

I understand that rule violations may result in removal without refund.

7. Photography & Media Release

I grant permission for RiverTown to capture photo/video footage of participants for marketing or promotional use.

8. Personal Property

I acknowledge that RiverTown is not responsible for lost, damaged, or stolen personal items.

9. Agreement Binding & Enforceable

I understand this agreement is legally binding, applies to all future visits unless revoked in writing, and covers all participants listed on this form.
