I understand by completing this application, it does not guarantee purchase of an Eastern Juniper Dachshund puppy.

Please provide 3 references (1 veterinary, if applicable). References must include their name, address, and phone number.

****Breeder has first pick if we choose to keep a pup in the program.

We do not allow in home visits at our home. The risk of disease is too great. Puppy pick up is out front. Initial below:

*****If you see a pup listed on the website text me. Tell me the pup you are interested in. If the pup is available, you will give a “pick-up promise” (I do not take deposits).

I ask that a family only make that “pick-up promise” when they are ready. Once the Pickup promise is made, that Puppy is removed from the website and the family starts receiving weekly pictures/vids until Pickup-day. This type of process puts the readiness power into the family’s hands.

Reminder: **** I DO NOT TAKE DEPOSITS. If you are unsure, please do not make a pick-up promise. This would be unfair to the pup when you do not show up on pick up day. They go home at 8 weeks, and this is not negotiable. If you have any questions do not hesitate to reach out.

Have you ever surrendered a pet to a rescue or shelter? If so, what was your reason?
