PLEASE READ CAREFULLY
APPOINTMENT INFORMATION
● The initial appointment will usually last approximately 40-75 minutes.
● Subsequent appointments can last anywhere from 30-60 minutes, depending on patient needs, at the therapist’s discretion.
● Please be ready to begin the session promptly at the scheduled time. When the client is late for the session, the client incurs the loss of time and payment for the full session is expected.
● In the event that you are unable to attend your appointment, we require at least 24 hours / one business day advance notice. When you schedule an appointment with our practice, that time is reserved for you. When you miss the appointment without calling to cancel within 24 hours, your practitioner does not have the opportunity to offer that time to someone else in need of services. It is our policy that patients are responsible for all appointments that they have scheduled. IF YOU FAIL TO GIVE US 24 HOURS / ONE BUSINESS DAY ADVANCED NOTICE, YOU MAY BE CHARGED FOR THE COST OF THE VISIT.
Fees for missed appointments and/or late cancellations are expected at or before the patient’s next scheduled appointment. Insurance does not cover these fees.
BILLING INFORMATION
Dr. Natalie Hoff Physical Therapy, PLLC dba First Off Wellness is NOT a participating provider with insurance companies, including Medicare. All charges are your responsibility from the date the services are rendered.
MEDICARE CLARIFICATION STATUS
Dr. Natalie Hoff Physical Therapy, PLLC dba First Off Wellness is not a Medicare Provider. We do not have a medical provider number with Medicare.
As a result, if you have Medicare, Dr. Natalie Hoff Physical Therapy, PLLC dba First Off Wellness:
● Is NOT permitted to treat Medicare recipients for fee-for-service.
● Is permitted to see you as a personal wellness client.
● Will not bill any payer for services either on behalf of Dr. Natalie Hoff Physical Therapy, PLLC dba First Off Wellness or on behalf of you. Medicare does not cover maintenance physical therapy.
● Will not issue any receipts that contain any diagnostic medical coding such as CPT or ICD-10 codest
