Board & Train Contract

This agreement, between the client (dog owner) and the trainer (Rush Pack Dog Training) pertains to the above mentioned dog.

The client agrees to pay the trainer a nonrefundable fee of $500 for the above listed board and train program to reserve the above board and train dates - unless previously discussed to waive.

SERVICE AGREEMENT

-The trainer agrees to provide the level of obedience the client wishes to achieve within reason of the duration of the training. The trainer will make every reasonable effort to help the client achieve training and behavior modification goals but makes no guarantee of the dog's performance or change in behavior once the dog is no longer in the care of the trainer.

-The client understands that he/she and members of the household must follow the trainer's instructions without modification, work with their dog daily as recommended, and constantly reinforce training that has been given to said dog. All training methods must be adopted and followed through to guarantee any change in the dog's behavior.

-The trainer is responsible for training the dog on all the agreed upon commands for as long as it takes for the dog to demonstrate clear understanding of all commands. The owner will then be taught how to handle the dog. The owner must follow all instructions given by the trainer. The owner understands and agrees that verbal and/or written instructions will be given to the owner by the trainer on how the owner can solve or curtail the behavior problems. The owner is responsible for the dog’s final outcome.

-The client fully understands the tools and methods that may be suggested to train and modify the behavior of the dog; they may include but are not limited to; martingale collar, flat buckle collar, prong collar, bark collar, remote training collar, head collar, and muzzle.

-Weekly one-hour training sessions with the client are offered at cost but not required during the duration of the program.

LIABILITY AGREEMENT

-Rush Pack Dog Training will endeavor to create as safe an environment as possible for the training and boarding care of the dog identified above and will offer only sound, safe and responsible training and care. The dog will be housed in a home environment.

-The client agrees, however, that all animals may exhibit unpredictable behavior and that Rush Pack Dog Training will not be liable for the actions of the dog that are outside of the control of the trainer.

-Rush Pack Dog Training will not hold the client responsible for or against any claims of injury, expense, cost, loss or damages caused by the gross negligence or misconduct of the trainer while the dog is in the sole custody, care and control of Rush Pack Dog Training.

-The client acknowledges that the trainer has no control over the dog when the dog is not in the sole custody, care and control of the trainer.

-The client acknowledges and agrees that owning any animal, including a dog, has inherent risks, including but not limited to the risk of dog bites to the owner or other family members.

PAYMENT & CANCELLATION POLICY

-Payment Policy: In order to reserve this board and train program, the client must submit a $500 nonrefundable deposit as well as the dates of the dog’s stay. The remaining balance is due PRIOR to the dog beginning their training program.

-Cancellation Policy: If the client chooses not to follow through with training, a refund for the deposit will NOT be issued. If the client has paid more than the above deposit – a refund MAY be issued if the client informs the trainer a month prior to their scheduled board and train.

PHOTO/VIDEO RELEASE

-I hereby assign and grant Rush Pack Dog Training the right and permission to use, reproduce, distribute, and publish photographs and videos of the dog at any time during the training class. Extra permission will be asked by the trainer to take,

post, share or publish photographs and videos containing the client’s face.

-I hereby release Rush Pack Dog Training from any and all liability resulting from such use, reproductions, distributions and publications of said photographs and videos.

-I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photographs and videos, without limitation at the discretion of Rush Pack Dog Training and I specifically waive any right to compensation I may have for any of the foregoing.

PICK UP/DROP OFF AGREEMENT

-The above dog will be picked up and dropped off by the above client unless other arrangements have been made. All board and train programs include a 1-2 hour “pickup” session which gives the trainer and client time to go over all training the dog has learned and for the trainer to correctly instruct the client on how to perform the same level of training at home.

-When bringing your dog for a board & train all Rush Pack Dog Training needs is your dog’s food and any medications your dog needs; including flea/tick and/or heart-worm (only if they need to be given to the dog during their stay).

-Please also bring their kennel/crate. You may also bring bedding/blankets for in their kennel - as long as they will not destroy them. Please do NOT bring any toys, water/food bowls or treats. Bones are allowed – no rawhides.

-When you arrive, you will hand off the dog to the trainer OR you will place your dog in their kennel/crate. You are NOT to hug, kiss, bend down or use a high-pitched voice at all. Not following these instructions can jeopardize the safety of the trainer.

-Upon arrival, your dog must be on a leash/collar setup from which they cannot slip out of. Your dog may be nervous, scared or excited and any of those things can lead to your dog escaping their collar and running off or around the property, which is a safety issue for your dog, the trainer and any other dogs or humans on the property.

ACTIVITY AGREEMENT

-Your dog will be staying in a home/kennel environment.

-Your dog will be spending a good amount of time in the outside world.

-Your dog may be taken into pet friendly public places. Your dog will be around things including grass, trees, dirt, concrete, natural objects, fencing, bushes, and may participate in swimming and structured interaction with other dogs/animals and other humans.

-Your dog will travel from location to location in my dog van, in a crash tested kennel. Any travel plans that are not local will be discussed with your prior to your dog ever going on a trip more than 30-40 miles.

-As the client, you acknowledge and understand that there are certain risks involved in dog training, boarding and care.

-You release, indemnify and agree to hold Rush Pack Dog Training harmless from any and all manner of damages, claims, loss, liabilities, cost, expenses, causes of actions or suits, whatsoever in law or equity (including; without limitation, attorney's fee and related costs) arising out of or related to the services provided by Rush Pack Dog Training.

The below signature serves as an agreement that this is a binding contract between the client (yourself) and the trainer (Rush Pack Dog Training) for the above selected training of your above listed dog.
