Grant of Rights
I, the undersigned, hereby grant View A Miracle 3D/4D Ultrasound, LLC, its representatives, employees, agents, successors, licensees, and assigns (collectively, “Company”), the irrevocable and unrestricted right to record, photograph, and/or videotape me during my visit at the Company’s studio, and to use, reproduce, publish, distribute, display, broadcast, and create derivative works from such recordings, photographs, and/or videos (collectively, “Media”) in any and all media formats, now or later developed, for any lawful purpose including but not limited to advertising, marketing, promotional, and educational purposes.
Waiver of Rights
I waive any right to inspect, approve, or claim compensation for the use of the Media. I release and discharge the Company from any and all claims, demands, or causes of action that I may now or in the future have arising out of or related to the use of the Media, including claims for invasion of privacy, right of publicity, defamation, or misappropriation.
Acknowledgment
I am over the age of 18 and have the full right and authority to enter into this Release. This Release shall be binding upon me, my heirs, executors, administrators, and assigns, and shall be governed by the laws of the State of New Jersey.
