Release from Liability, Waiver of Claims, Conduct Policy, Likeness Release, and Indemnification Agreement

In consideration of the undersigned, any minor children, spouse, parents, family members, or others under the control or supervision of the undersigned (collectively, the “Participants”) receiving permission to participate in the activities offered by Bluemont Local LLC (doing business as Bluemont Local) including, but not limited to, being present at, and the use of the equipment, facilities, and programs of, the entertainment center operated by Bluemont Local LLC (doing business as Bluemont Local) (the “Facility”), which contains and offers activities including, but not limited to, climbing, swinging, crawling, jumping, running, games, elevated net and ninja courses, climbing challenges, soft play, dancing, and virtual reality equipment (collectively, the “Activities”), the undersigned, on behalf of the undersigned, the Participants, and the heirs, executors, administrators, legal representatives, successors, and assigns of all Participants (collectively, the “Releasors”), hereby agrees as follows:

Please include the additional caregiver who will accompany the children during the visit or future visits.
