After receiving your form, we will reach out to approve your theme. Please do not purchase or begin planning your theme until you receive written approval from our committee or staff.

When submitting this form, please acknowledge that you are responsible to provide the following items:

New in box, LED prelit artifical tree between 6' and 8' (Use of any tree other than a traditional Christmas tree between 6' and 8' must be approved by the Feztival of Trees Committee prior to the event)

New 15' power cord that meets electrical code

Printed inventory list of all prizes along with the value of each prize
