Our organization agrees to use the Items Request Form for all requested items outside of the regular scheduled weekly pick-up.

Our organization agrees to credit Katie Grace Foundation's product donations, when appropriate.

Our organization acknowledges and agrees to provide relevant organizational data upon request, as needed for verification, compliance, or reporting purposes
