2026 Flicky Stick Pole Vault Pole Rental Agreement & Safety Waiver

A parent/guardian signature is required.

All pole rentals, returns, and swaps must be conducted during normal hours of operation at the Flicky Stick Pole Vault facility unless otherwise arranged with a member of the Flicky Stick Pole Vault staff. A pole rental agreement and safety waiver are required for all rentals.

Subscription & Late Fee Policy

Meet Pole Rentals

Meet pole rentals are free 2-3 day rentals (available to select club members only). Should an athlete fail to return his/her pole by the end of the rental period, a $35/week late fee will be automatically charged to the credit card on file until the pole is returned to the Flicky Stick Pole Vault facility. Should the payment fail for any reason, a late fee of $35/week will be due upon returning the rental pole/equipment.

7-Day Pole Rentals

All 7-day pole rentals (available to club members only) are on an automated subscription. Should an athlete fail to return his/her pole by the end of the rental period, the subscription will renew and be charged to the credit card on file until the pole is returned and the subscription canceled. Should the payment fail for any reason, a late fee of $35/week will be due upon returning the rental pole/equipment.

30-Day Pole Rentals

All 30-day pole rentals are on an automated subscription. Should an athlete fail to return his/her pole by the end of the rental period, the subscription will renew and be charged to the credit card on file until the pole is returned and the subscription canceled. Should the payment fail for any reason, a late fee of $35/week will be due upon returning the rental pole/equipment.

90-Day Pole Rentals

All 90-day pole rentals are on an automated subscription. Should an athlete fail to return his/her pole by the end of the rental period, the subscription will renew and be charged to the credit card on file until the pole is returned and the subscription canceled. Should the payment fail for any reason, a late fee of $35/week will be due upon returning the rental pole/equipment.

If a pole is not returned within 30 days of the due date, the credit card on file will be charged the full replacement cost of the pole including shipping in addition to the previously charged subscription and/or late fee payment(s). Should the payment fail for any reason, an invoice for the amount due will be sent to the email address on file. If payment is not received within 30 days, Flicky Stick Pole Vault shall seek to enforce all of its available rights under applicable law to collect the full amount owed.

Damage & Negligence Policy

Damage Policy

All poles will be inspected upon return. Any damage to the pole/equipment will result in a damage fee of $75.00 up to the full replacement cost of the pole/equipment. If a pole is returned broken the credit card on file will be charged the full replacement cost of the pole including shipping in addition to the previously charged subscription payment(s).

Negligence Policy

Additional charges will be applied to the credit card on file for any pole/equipment dropped off outside the facility and left unattended. Should the pole be lost/stolen due to improper drop-off outside the facility, the full replacement cost plus shipping costs will be charged to the card on file. Should a pole be returned damaged or broken due to negligence, the credit card on file will be charged a minimum damage fee of $75.00 up to the full replacement cost of the pole including shipping in addition to the previously charged subscription payment(s).

Credit Card Authorization

I acknowledge the above statement and authorize Flicky Stick Pole Vault to charge these amounts (subscription fees, late fees, and/or damage and/or replacement costs) to the credit card provided herein should these circumstances occur. I agree to pay these charges in accordance with the issuing bank cardholder agreement.

Safety Information

Pole vaulting involves risk of serious injury, including death, caused by hazards inherent in the sport. Participation in the sport of pole vaulting, no matter what type equipment you are using or the event or program that you have chosen to participate in, is a voluntary act on the part of the participant and where applicable the participant’s parents or guardians.

PARENT(S)/GUARDIAN(S): Only allows your child or children) to participate in the sport of pole vaulting if he/she:

• Appreciates the risks involved in the sport;

• Knows his/her own capabilities and limitations regarding the sport of pole vaulting;

• Is capable of exercising common sense

Release & Waiver of Claims

In consideration for being allowed to participate in the sport of pole vaulting at Flicky Stick Pole Vault classes or events, or to rent pole vaulting poles from Flicky Stick Pole Vault, the Participant, and the Participant’s parent or legal guardian if the Participant is a minor, do hereby agree, to the fullest extent permitted by law as follows:

TO WAIVE ALL CLAIMS that they have or may have against Flicky Stick Pole Vault, any coaches, or volunteers, arising out of the inherent risks of the activity of pole vaulting.

TO ASSUME ALL RISKS INHERENT IN THE SPORT OF POLE VAULTING; and TO RELEASE Flicky Stick Pole Vault, the facility, its owners, affiliates, officers, directors, employees, agents, members, coaches, volunteers, and all distributors of equipment from all liability from any loss, damage, injury, or expense that the Participant or his next of kin may suffer, arising out of the inherent risks of participation in the sport of pole vaulting.

Equipment Rental & Release of Liability and Waiver of Claims

I accept for use the equipment rented to me by Flicky Stick Pole Vault (the “equipment”) and accept full responsibility for its care while it is in my possession. I will not allow anyone not a party to this agreement to use the Equipment, and will indemnify Flicky Stick Pole Vault, against any claims that arise from any equipment use by a non-party to this agreement. I will be responsible for the replacement, at full retail value, of any Equipment which is not returned, and I will be responsible for the costs of repairing or replacing any damaged Equipment, other than damage caused by normal wear and tear.

POLE VAULT POLES: I understand that the pole vault poles are designed to be used only by persons who weigh less than the rating of the pole. Use by person whose weight exceeds the limit of the pole can cause the pole to break, resulting in juries to the pole vaulter. If I choose to use a pole with a rating less than my weight, I accept the risk of injury. I understand that even poles with ratings in excess of the weight of the pole vaulter may break, resulting in injuries to the pole vaulter. This is a risk inherent in the sport of pole vaulting and I accept this risk.

INSPECTION OF POLES: I agree that it is my responsibility to examine any pole that I use for damage (e.g., cracks or chipping) or unsafe condition before renting or using such pole. I understand that Flicky Stick Pole Vault cannot inspect and monitor the condition of poles once they are in my possession. Further, I agree that once I have inspected the pole, found it satisfactory, and taken possession of it, Flicky Stick Pole Vault bear no responsibility for its condition or any resulting injuries or harm if the pole is subsequently used when it is damaged or unsafe.

I hereby agree to the fullest extent permitted by law, as follows:

TO WAIVE ALL CLAIMS that I have or may have against Flicky Stick Pole Vault, and all distributors of the equipment caused by the use of the Equipment and/or the inherent risks of the sport of pole vaulting;

TO ASSUME ALL RISKS INHERENT IN POLE VAULTING;

TO RELEASE Flicky Stick Pole Vault and all distributors of the Equipment from all liability for any loss, damage, injury or expense I (or my next of kin) may suffer, caused by the use of the Equipment and/or the inherent risks of the sport of pole vaulting;

TO INDEMNIFY and hold harmless Flicky Stick Pole Vault, all distributors of the Equipment for any loss or damage including any that results from claims or lawsuits for personal injury, death, or property loss and damage arising from any use of the Equipment

I FURTHER AGREE to submit all claims against Flicky Stick Pole Vault and all distributors of the rental Equipment used by me to arbitration.

I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I MAY BE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

Pole Care Instructions

• NEVER place pole(s) on the ground unless properly stored inside a sturdy and secure pole bag

• NEVER toss or throw pole(s) in a forceful or aggressive manner

• NEVER step on pole(s) or place heavy/destructive items on top of pole(s)

• NEVER hit or strike pole(s) with or against other item(s)

• NEVER pull down on pole(s) in a forceful or aggressive manner

• NEVER allow another individual or organization to use or transport your pole(s)

• ALWAYS transport pole(s) safely and securely with proper equipment

• ALWAYS store pole(s) indoors or inside a sturdy and secure pole bag while not in use

• ALWAYS look after pole(s) at practice and/or meets especially when traveling

• ALWAYS know the whereabouts of your pole(s) during your rental
